1. Put cover.png or cover.jpg in the same directory as .gdi file
2. Did you even save it? You need to either check the box to save it or click the button at the bottom of the settings page.
3. You need to create an AVI container with MPEG4 and PCM audio. See the conversion examples at the bottom of this article:

2. Yes, I have gotten tired of trying, clicking in "Save preset" AND in "Save" botton at the bottom. (all in DS 4.0.3)
